Health insurance for a residence card and visa in Poland: types, mistakes, how to do it right
Updated for 2026
For almost every basis of a residence card you have to confirm health insurance – proof that your treatment costs in Poland are covered. Which option fits depends on your basis: work, study, family, business, temporary protection. This is not one document for every case, but different types of insurance for different cards and visas.
This page is a navigator: which insurance is tied to which basis, how NFZ, a private policy and employer cover differ, what to check in the policy, and which insurance mistakes cause problems when you apply. If you want to work through the whole document set, start with the overview of the Poland residence card.
Why insurance matters for a residence card
When reviewing your application, the urząd (voivodeship office) checks not only your basis of stay but also how you will cover medical costs. If there is no proof of insurance, or the policy does not match your basis, this can trigger a request for extra documents and sometimes affect the decision.
The key point is continuity. Insurance must be valid on the day you apply and for the whole processing period, not run out somewhere in the middle. A gap in cover is one of the typical reasons a case drags on.
Types of insurance: NFZ, private, travel, employer
Insurance for foreigners in Poland comes in several forms, and they are not interchangeable. A travel policy for a visa and full health insurance for a residence card are different things.
| Type of insurance | What it is | Who it usually fits | What to check |
|---|---|---|---|
| NFZ (public, Narodowy Fundusz Zdrowia) | State health insurance, most often through official employment and contributions. You can also sign a voluntary NFZ agreement | People employed under a contract or running a business, for whom contributions are paid; certain categories by status | Cover is tied to employment; it ends after you leave the job |
| Private insurance | A policy from a private insurer | Study, waiting for a decision, bases without NFZ | Cover amount, validity period, match to your basis |
| Travel / visa | A short-term policy for the trip | Visa, short stay | Usually does not cover long-term stay and is not suitable for a card |
| Employer | NFZ through employment or an extra policy from the company | People on an employment contract | The contract type affects whether contributions are paid – worth checking |
Which insurance you need for which card or visa
Each basis has its own typical source of insurance. Below is a short matrix, and under it a breakdown with links to the relevant pages.
| Basis / card | Insurance usually needed |
|---|---|
| Temporary residence | Policy or NFZ cover in the document set with your basis |
| Permanent residence | Its own requirements when moving to permanent stay |
| EU long-term resident | Separate conditions for the long-term EU resident |
| Study | Private or voluntary policy |
| Work | NFZ through official employment |
| Blue Card | Its own requirements for highly qualified specialists |
| Family | Cover for the applicant and family members |
| Temporary protection, CUKR | From 2026 – general rules: private policy or official employment |
For a temporary residence card, proof of insurance is usually part of the document set with your basis. When moving to permanent residence and for the EU long-term resident card the requirements differ.
When applying for a card by work, study or family reunification the source of cover differs: for work it is usually NFZ through official employment, for students a private policy. Highly qualified specialists use the Blue Card, with its own requirements.
For Ukrainian citizens, general rules apply from 2026: free public healthcare is available to people in official employment (with ZUS contributions) and to protected categories – children under 18 and pregnant women. CUKR card holders move to the general rules for foreigners and need a standard private policy or official employment.
What to check in your insurance
Before you apply it helps to run through a short list. The mistakes here are usually not about having insurance at all, but about the details.
- The validity period covers filing and processing with no gaps between your basis and the policy
- The cover amount matches the requirements of your basis
- It is clear who issued the policy, and the document is accepted at filing
- Cover is valid on Polish territory
- The type of insurance matches your basis: a travel policy does not replace health insurance for a card
Common mistakes and refusals because of insurance
- A gap in the period: the old policy has ended and the new one has not started
- Cover amount too low for the requirements of your basis
- A travel policy instead of full health insurance
- The policy expired by the time the case was reviewed, although it was valid at filing
- NFZ stopped after you left a job and new cover was not arranged
How to arrange insurance correctly and confirm it when filing
- Identify your basis and which type of insurance is tied to it
- Take out a suitable policy or confirm active NFZ cover
- Check the validity period and cover amount for your basis
- Prepare the document confirming insurance and proof of payment for the policy
- Attach them when filing or upload them to the MOS 2.0 system for online filing
When you need a specialist
With a standard basis and clear employment you can sort out insurance yourself. Help is more often needed when the situation is non-standard: several bases, a change of employer mid-case, a gap in cover, a risk of refusal, or an already received request for extra documents.
MigrantExpert will check whether your insurance matches your basis and tell you what to prepare for filing.
Frequently asked questions about insurance for a residence card
Do you need insurance for a residence card
For most bases, yes. You usually need a private health policy with cover from EUR 30,000, valid for the full processing period, or NFZ cover. The exact requirement depends on your basis and the type of card.
Does NFZ through work count
If you are officially employed and contributions are paid for you, NFZ cover usually meets the insurance requirement. Check that contributions are actually being paid and that cover has not lapsed.
What if I do not work officially
Then NFZ through employment does not apply, and people usually take out a private policy for their basis. It must match the requirements and have no gaps in dates.
What insurance do Ukrainians on temporary protection need
From 2026, free public healthcare is available to people in official employment (with ZUS contributions) and to protected categories – children under 18 and pregnant women. CUKR card holders move to the general rules: a standard private policy or official employment is needed.
How do you confirm insurance when filing in MOS
The confirming document is added to the set and uploaded to the system when filing, together with proof of payment for the policy.
Can you be refused because of insurance
An insurance problem – a gap in the period, the wrong type of policy, insufficient cover – can lead to a request for extra documents and affect the case. That is why the policy is checked in advance.
