Required Documents for a TRC in Poland: The Ultimate 2026 Checklist

Your final self-check before submitting via MOS 2.0 — universal base requirements, purpose-specific documents, sworn translation rules and how to organise your file.

Golden rule: a single missing document triggers a Wezwanie letter, pauses your case, and adds weeks to your waiting time. Work through this checklist section by section before you submit — not after.

Universal Base Requirements (Every Applicant)

Every TRC applicant — regardless of purpose, nationality, or voivodeship — must include the following items. There are no exceptions.

  • Completed wniosek — filled out online via MOS 2.0
  • 1 digital photo — colour, white background, full face, no glasses, 684×883 pixels, taken within the last 6 months
  • Passport — original to show at every appointment (do wglądu) + photocopies of all pages
  • Stamp duty receipt (opłata skarbowa):
    • 440 PLN — TRC by Work or Blue Card
    • 340 PLN — all other purposes (study, family, etc.)
    • 100 PLN — card printing (50 PLN for students and children under 16)
  • Health insurance — see details below
  • Proof of accommodation — see details below
  • Email of employer / university — for Annex nr 1 verification
  • Sworn translations of all non-Polish documents
Payment note: stamp duty must be paid by bank transfer from a Polish bank account. All confirmations must be in Polish and submitted at the time of application. Pay to the correct account of your local municipality (gmina).

Health Insurance: Accepted Proofs

✅ ZUS registration proof

ZUS ZUA or ZUS ZZA form, or a printout from the ZUS portal confirming active social insurance contributions. Applies if you are employed under umowa o pracę or umowa zlecenie with ZUS contributions deducted.

✅ Private health insurance

Must cover medical treatment in Poland with a minimum coverage of 30,000 EUR. Bring the original policy and a copy. Policies covering only emergency care or excluding in-patient treatment will not be accepted.

✅ NFZ voluntary contribution

Confirmation of voluntary NFZ contribution — applies to students or those not covered by employer ZUS.

Proof of Accommodation: Accepted Proofs

🏠 Rental agreement (umowa najmu)

Signed by both parties, showing your name and address. Should be current and valid. Inspectors look unfavourably on agreements expiring well before your requested permit period ends.

🏠 Property ownership document

If you own the property where you live.

🏠 Owner declaration (oświadczenie właściciela)

Written declaration from the property owner confirming they provide accommodation to you.

Purpose-Specific Documents

TRC by Work — additional documents
  • Załącznik nr 1 (Annex 1) — completed and signed solely by your employer (not by you). In the application form you provide the employer's email — they receive and fill in Annex 1 digitally and have 30 days to submit it. Without a correctly completed Annex 1, the Urząd will not process your application.
  • Employment contract (umowa o pracę / umowa zlecenie / umowa o dzieło) — current, signed by both parties, specifying position, salary, and duration. Bring original + one copy.
  • Proof of ZUS contributions — ZUS printout or employer statement confirming active social insurance registration.
  • KRS or CEIDG printout confirming the employer's legal status (free of charge online).

TRC for Students — additional documents
  • Certificate of enrolment (zaświadczenie o przyjęciu / kontynuacji studiów) — issued by your university, dated within the last 30 days, confirming faculty, year of study, and study mode (full-time preferred).
  • Proof of tuition payment — receipt or bank transfer confirmation for the current academic year.
  • Bank statement — demonstrating sufficient funds. Formula: (1,010 PLN + rent) × 15 + 2,500 PLN.
  • If on a scholarship: official scholarship award letter confirming amount and duration.

TRC for Family Reunification — additional documents
  • Marriage certificate (if applying as a spouse) — original or certified copy with a sworn Polish translation.
  • Birth certificate (if applying for or as a child) — same sworn translation requirement.
  • Sponsor's legal status proof — Polish citizen's dowód osobisty, or the foreign national sponsor's valid TRC and decision.
  • Proof of cohabitation — joint rental agreement, utility bills, or a co-habitation declaration.

The Sworn Translation Rule

⚠️ Any document not originally issued in Polish must be translated by a Polish sworn translator (tłumacz przysięgły) — a certified professional registered with the Polish Ministry of Justice. This applies to birth certificates, marriage certificates, foreign diplomas, court documents, and any other official foreign document.
❌ NOT accepted
  • A notarised translation prepared in your home country
  • A translation by a bilingual friend
  • A standard translation agency without sworn status
  • A notary who is not a registered Polish tłumacz przysięgły
✅ Accepted
  • Translation bearing the stamp and signature of a tłumacz przysięgły registered in Poland
  • Cost: typically 50–150 PLN per page depending on language and complexity
  • Find registered translators at the Polish Ministry of Justice website

How to Organise Your File

At every appointment, bring originals of every document (the inspector must inspect them) and submit copies in your file. Copies do not need to be notarised — the inspector verifies them against originals.

Recommended file order:
  1. Completed wniosek
  2. 1 digital photo confirmation
  3. Government payment confirmations (440/340 PLN + 100 PLN)
  4. Passport copies (all pages)
  5. Employer / university email (for Annex nr 1)
  6. Employment contract / university certificate / family documents (copy + original to show)
  7. Health insurance proof
  8. Accommodation proof (rental agreement, etc.)
  9. Sworn translations of all foreign-language documents

Frequently Asked Questions (FAQ)

❓ What happens if I forget a document when submitting?

The Urząd Wojewódzki will issue a Wezwanie — a formal letter requesting the missing materials. You typically have 7 to 14 days from the date on the letter to supply the missing items. If you respond in time, your application continues normally. If you do not respond, your case may be left without consideration (pozostawienie wniosku bez rozpoznania) or result in a negative decision.

Always check your physical post box regularly after submission — Wezwanie letters arrive by post and missing one is costly.

❓ Does my rental agreement need to be valid for a specific period?

There is no legally mandated minimum duration. However, inspectors assess whether your accommodation situation is stable and consistent with the permit period you are requesting. If you are applying for a 3-year TRC but your rental agreement expires in 2 months, the inspector may ask questions.

Practical guideline: aim for a rental agreement covering at least the first year of the requested permit. If your contract is short-term, include a letter from your landlord confirming the long-term nature of the arrangement, or provide several previous agreements showing continuity of residence at the same address.
